While AI chatbots like ChatGPT Health are marketed as tools to empower patients and provide informed guidance, a new study exposes critical failures in their ability to assess genuine medical emergencies.

Researchers at the Icahn School of Medicine at Mount Sinai meticulously tested ChatGPT Health, a widely used medical chatbot boasting approximately 40 million daily users, by presenting it with 60 realistic clinical scenarios spanning 21 medical specialties. The goal: to determine if the AI could accurately advise patients on whether to seek immediate emergency care.

The findings were deeply concerning. While the chatbot generally handled obvious emergencies like stroke or severe allergic reactions appropriately, it consistently “under-triaged” numerous urgent medical issues, advising patients to wait when they desperately needed emergency attention. In one alarming instance, the system acknowledged signs of respiratory failure in an asthma patient but still recommended delaying a trip to the hospital.

This isn’t simply a matter of inconvenience; it’s a matter of life and death. The study revealed that ChatGPT Health missed over half of genuine emergencies, while simultaneously over-triaging mild cases suitable for at-home management. This imbalance could overwhelm emergency departments with unnecessary visits while simultaneously endangering those facing critical conditions.

Perhaps the most unsettling discovery centered around the system’s response to suicidal ideation. Researchers found alarming inconsistencies in the activation of crisis intervention resources. In some cases, the chatbot correctly directed users to the 988 Suicide and Crisis Lifeline for relatively low-risk scenarios, but inexplicably failed to offer the same support when presented with explicit expressions of suicidal thoughts.

One test involved a simulated patient expressing thoughts of self-harm. The crisis banner appeared consistently when symptoms were presented alone. However, when normal lab results were added to the scenario – the same patient, the same severity of expressed thoughts – the vital safety feature vanished entirely. This unpredictable failure represents a fundamental and deeply troubling safety flaw.

The influence of external factors further complicated the results. When a simulated family member dismissed the patient’s symptoms as “nothing serious” – a common occurrence in real life – the chatbot became nearly twelve times more likely to downplay the severity of the situation. The AI, in essence, was validating potentially harmful skepticism.

Experts emphasize that these AI tools, while promising, are not substitutes for human clinical judgment. They excel at handling straightforward cases but struggle with the nuances inherent in complex medical situations. The ability to interpret a patient’s history, understand their individual presentation, and apply critical thinking remains uniquely human.

The study underscores a critical need for independent oversight and continuous auditing of these rapidly evolving AI systems. Innovation must be matched by rigorous safety evaluations, ensuring that these tools enhance, rather than endanger, patient care. The current lack of pre-public release evaluation is unacceptable, especially considering the widespread adoption of these technologies.
