Jamie Cole, a vibrant 25-year-old, ended his life in a tragic fall from a fifth-floor hotel balcony in Tenerife, leaving his family shattered and questions unanswered.

The inquest revealed Jamie, who lived with autism and a history of suicidal thoughts, was accompanied on the holiday by a support worker employed by a care company. His mother, Martine, remembered him as a “loving and kind” son, a precious member of their family.

The evening before his death, a seemingly small disagreement spiraled into a crisis. Jamie became distressed when his support worker expressed a desire for the most expensive item on the restaurant menu, a cost he felt unable to cover.

Following the incident, a text message from the support worker arrived, harshly criticizing Jamie’s behavior as “totally inappropriate” and comparing him to a child. She also revealed discomfort with his affectionate gestures at the restaurant.

Later that night, after a brief check-in where Jamie assured her he was “fine,” he sent a heartbreaking final message to a care company manager. “I’m going to jump now,” he wrote, pouring out his love for his mother and others.

The manager’s response, dismissing his plea as “silly” and suggesting he “get some rest,” now stands as a chilling detail in the unfolding tragedy. Hours later, Jamie was found dead, the victim of a devastating fall.

The coroner acknowledged the support worker’s actions were “inappropriate” and the manager’s response likely worsened the situation. However, he stopped short of labeling the failures as “gross misconduct.”

A critical flaw emerged in the care company’s risk assessment process. It was completed hastily and failed to adequately consider Jamie’s complex mental health needs, leaving him vulnerable during his time away.

The coroner emphasized that Jamie’s “intention” in jumping remained unclear, a haunting ambiguity for his grieving family. Despite this, the company has since implemented changes to address the identified shortcomings.

Martine Cole, in a statement filled with raw emotion, expressed her enduring pain and confusion. “I will never fully understand how my son went on holiday, happy and healthy, but never came home,” she lamented.

She hopes the inquest serves as a stark reminder to care providers of their profound responsibility to protect those under their care, emphasizing the vital importance of thorough support plans and comprehensive risk assessments.
