A citizen journalist recently brought a specific case in Minnesota to light, documenting potential fraud within state-funded daycare programs. His video quickly went viral, sparking outrage and demanding answers from officials.

While that investigation is important, it barely scratches the surface. Healthcare spending, particularly within Medicaid, dwarfs any other area of government waste, fraud, and abuse, especially in states with expanded coverage and generous federal funding.

Local hospitals could have provided care, yet the journey to another state was deemed necessary.

The questions are stark: What specialized care justified this extraordinary expense? How can a single appointment incur such a massive transportation cost? And, most importantly, who ultimately bears the burden of these expenses?

In 1988, New York spent $10 billion on Medicaid for a population of 18 million. Today, with a population of 20 million, the state spends approximately $120 billion – four times as much per person, even accounting for inflation.

Recent spending has actually *accelerated*, fueled by the understanding that the federal government will continue to provide a financial safety net. This creates a perverse incentive, rewarding unchecked growth and discouraging fiscal responsibility.

Despite this massive investment, there’s no evidence to suggest that New Yorkers are healthier or receive superior care compared to residents of states with more restrained Medicaid spending. The system is financially unsustainable and potentially detrimental to patient care.
