A seemingly minor detail – loose screws – brought Toronto’s bustling Union Station to a standstill earlier this month, disrupting the commutes of thousands.

The February 2nd derailment of a westbound GO train wasn’t caused by a catastrophic failure, but by fatigue and breakage of lag screws securing the tracks. These screws, meant to fasten wooden plates, were discovered to be held in place by only two fasteners instead of the required four.

The train derailed on a curved section of the line as it departed the station, damaging critical signalling infrastructure and two track switches. Four platforms were immediately blocked, triggering widespread service disruptions for both GO Transit and UP Express.

Remarkably, no one was injured in the incident, but the fallout extended for days, highlighting a systemic issue that had persisted for years. An internal review revealed the updated safety standard requiring four screws per tie had been in place since 2016, yet wasn’t consistently enforced.

Transit officials immediately launched a comprehensive inspection of the rail corridor surrounding Union Station, identifying and correcting the remaining instances of non-compliance. Inspections have been significantly heightened to prevent a recurrence.

The head of the transit agency publicly apologized for the disruption, acknowledging shortcomings in both infrastructure maintenance and customer communication during the crisis. A key lesson learned focused on improving the connection between on-train staff and customer service representatives.

As a direct response to the inconvenience caused, the transit agency is proactively issuing refunds to commuters who travelled through Union Station on February 2nd and 3rd, with additional refunds for specific trips on February 4th. Customers will receive these refunds automatically, requiring no application process.

While the total cost of repairs and refunds remains to be calculated, officials have committed to a full public accounting of all expenses related to the derailment and its aftermath. The incident serves as a stark reminder of how even the smallest component can have a significant impact on a complex transportation system.
