Senator Tom Cotton is pressing the Food and Drug Administration to investigate the potential infiltration of illegally sourced ingredients from China into these medications.

The concern isn’t simply about efficacy; it’s about public health and national security. Cotton’s letter to the FDA Commissioner highlights the vulnerability of the U.S. pharmaceutical supply chain and the very real possibility that dangerous, unregulated ingredients are ending up in drugs people are taking to improve their health.

Recent data paints a disturbing picture. Authorities intercepted 195 illegal shipments of active pharmaceutical ingredients between September 2023 and January 2025, with a significant portion – roughly 60 – originating from China and Hong Kong. These ingredients are believed to be destined for compounded weight loss medications.

The scale of the potential problem is staggering. Estimates suggest that by January 2026, as many as 1.5 million Americans could be unknowingly using compounded weight loss drugs containing these questionable ingredients. This raises serious questions about the oversight of these medications.

These ingredients are frequently found in compounded versions of GLP-1 drugs, often marketed as lower-cost alternatives to established medications like Ozempic and Wegovy. The appeal of affordability, however, may come at a hidden and potentially dangerous cost.

The Department of Health and Human Services recently took action against telehealth company Hims & Hers, referring them to the Justice Department over concerns about a non-FDA-approved weight loss drug. This move signals a growing awareness of the risks associated with these unregulated alternatives.

Following mounting pressure, Hims & Hers voluntarily removed its weight loss pill from the market, a step Cotton acknowledges but believes is only the beginning. He is urging the FDA to expand its investigations to encompass all entities potentially exposing patients to these dangerous ingredients.

The situation demands a thorough and swift response. The health and well-being of millions of Americans may depend on uncovering the full extent of this issue and securing the pharmaceutical supply chain against these threats.
