In rare cases, extreme muscle damage can lead to rhabdomyolysis, a condition that threatens kidney function by releasing large amounts of metabolic waste into the bloodstream. Although uncommon, it requires prompt medical attention.
Studies have documented only a few dozen rhabdomyolysis cases over several years in a major urban hospital, underscoring its rarity among the exercising population.
Individuals most vulnerable to exercise‑induced rhabdomyolysis include those who are dehydrated, exercise in high heat, engage in excessively intense or prolonged sessions, are new to physical activity, or use certain medications such as non‑steroidal anti‑inflammatory drugs.
Additional triggers can involve severe muscle injuries, specific drugs, or underlying health disorders that compromise muscle integrity.
Warning signs may emerge days after a workout and include intense muscle pain, swelling, marked weakness, dark‑colored urine, and extreme fatigue. These symptoms warrant immediate evaluation in an emergency or urgent‑care setting.
Medical assessment typically involves blood tests for creatine kinase levels, repeated over time to confirm muscle breakdown and guide treatment to protect kidney function.
For ordinary DOMS, recovery relies on adequate sleep, nutrition, and time. No medication can eliminate the soreness outright; the body must heal naturally.
Active recovery strategies—such as gentle walking, low‑intensity cardio, self‑massage with a foam roller or lacrosse ball, moderate stretching, and applying heat—can improve comfort while the muscles repair.
