A startling investigation has uncovered a controversial practice within California’s Medi-Cal program. Taxpayer funds are being used to provide comprehensive gender-affirming care – including hormone therapy, breast implants, and surgical preparation – to individuals residing in San Francisco shelters.
These shelters are not only providing housing, but are also serving as access points to extensive medical procedures for individuals who have entered the country illegally. The facilities house people from Honduras, Mexico, El Salvador, and Venezuela, operating independently of federal immigration enforcement.
At the MSC-South facility, San Francisco’s largest shelter with a $66 million city contract, reporters found multiple residents receiving state-funded treatments. The shelter reportedly does not inquire about immigration status, creating a pathway to services regardless of legal standing.
“Lyca,” a Honduran resident identifying as a transgender woman, openly acknowledged receiving cross-sex hormone therapy through Medi-Cal, stating simply, “Tengo Medi-Cal.” Her words revealed a system where access to care is readily available, despite her undocumented status.
Another resident, “Alondra,” also from Honduras and seeking asylum, was offered financial assistance for private housing. He declined, however, citing concerns about affording the ongoing costs beyond the initial support provided by the city.
The investigation revealed that at the Embarcadero SAFE Navigation Center, a resident named “Jacqueline,” from Mexico, freely discussed receiving hormone treatments and breast implants fully covered by Medi-Cal. She emphasized that undocumented individuals are eligible for these services, requiring only therapy and hormone prescriptions.
“Jacqueline” further stated she was awaiting “bottom surgery,” a complex procedure involving potential castration and the creation of female genitalia, highlighting the full spectrum of care being offered. The process, she explained, is accessible even without legal documentation.
The Taimon Booton Navigation Center, specifically designed for transgender, gender non-conforming, and intersex individuals, also houses a population of male-to-female transgender individuals seeking similar medical interventions. A clear pattern emerged of individuals seeking refuge and medical care within the state.
Word has spread throughout transgender communities in Latin America, painting California as a sanctuary where shelter, hormones, and surgeries are available without question. This has fueled a migration towards the state, driven by the promise of comprehensive medical support.
Under the leadership of Governor Gavin Newsom, Medi-Cal expanded its coverage to include “full scope” care for undocumented individuals, classifying gender-affirming procedures as “medically necessary.” This policy change has directly enabled the current situation.
Shelter employees confirmed they have been instructed not to cooperate with Immigration and Customs Enforcement (ICE), further solidifying the separation between housing and immigration enforcement. This creates a unique environment where medical services are prioritized regardless of legal status.
Requests for comment from the California Department of Health Care Services, which oversees Medi-Cal, and the shelters’ executive directors went unanswered, leaving many questions surrounding the program’s implementation and oversight unresolved.
